How Braces Work

Brackets can be fabricated using several methods, including stamping and extrusion. In stamping, metal sheets are flattened and shaped into a bracket. Extrusion is similar but forms long pieces cut to the proper size. Both processes are cost-effective and produce large quantities of metal brackets quickly.

In the United States, several laws and regulations govern the production of metal brackets. These include ISO 9001 and the Occupational Safety and Health Administration (OSHA). In addition, companies that manufacture metal brackets must comply with state and federal regulations regarding using hazardous substances in their products.

In addition to enhancing facial symmetry and beauty, braces also provide many health benefits. For instance, crooked teeth and misaligned jaws can cause chewing issues that affect digestion. These can lead to intestinal irritation and even bone loss in the jaws. Braces help realign these problems, ensuring your body can digest foods properly and reduce the risk of long-term dental issues.

Overcrowded or crooked teeth can leave space for food particles to become trapped, leading to bacteria buildup. This can be a major cause of bad breath. The elastics that hold wires and brackets together come in different colors, sizes, and force categories. Keeping a supply of elastics on hand is important, as these must be changed often. It’s best to get into the habit of always carrying a few spares in your purse or backpack so you can replace them as soon as they break (if needed) without waiting until you’re home. Consider getting an elastic placer with two differently-shaped hooks that allow you to quickly and easily insert and remove elastics. This can be especially helpful for people with shaky hands or prone to misplacing their elastics.

Spacers are rubber rings or small metal appliances that create spaces between back teeth before applying bands. They are placed between your teeth at the second orthodontic appointment. Usually, it’s a week or two before your braces go on. This helps ensure the molar bands have enough room to fit comfortably between your back teeth. Some patients feel pain or pressure when fitting as the orthodontist inserts the spacer. This sensation is normal and should subside shortly after the procedure. Taking some over-the-counter pain relievers is a good idea if you are feeling discomfort.

After a few days, you will notice that the spacers have created spaces between your teeth. This may cause your gums to ache slightly, or you may feel a slight pressure between your teeth and gums, similar to having a piece of food stuck between your teeth. This discomfort is normal and a sign that the spacers are working!

The orthodontist will remove the spacers once the gap is created between your back teeth. They will then check that the gap is wide enough to apply the molar bands and will clean your teeth. At this point, you are ready for your braces!

While wearing your spacers, avoiding hard or sticky foods like taffy, gum, caramel, corn on the cob, and raw apples is a good idea. These types of foods can cause your teeth to become irritated, and they can also break wires or loosen brackets. Similarly, brushing your teeth thoroughly and regularly, especially after eating, is important to prevent plaque buildup around the brackets.

The archwire runs through the brackets and puts pressure on your teeth to guide them into their final straight position. This direct, consistent pressure is important because it causes the formation and disappearance of bone cells around the root of each tooth, which eventually moves the tooth into its desired position. In addition to applying direct pressure, the orthodontist may bend the archwire at certain points along its length. This is done to apply more or less pressure to specific crooked teeth or to help the roots of particularly resistant teeth take on more straightening force. Traditional orthodontic wires are made of metal alloys, most commonly stainless steel. These rigid archwires have the advantage of being easy to handle and do not corrode in the mouth. They can be made more flexible by bending loops into them or by winding several wires of small diameter around each other (coaxial and multi-strand). Recently, some manufacturers have promoted superelastic nickel-titanium (NiTi) archwires. These are said to have more “shape memory” and thus deliver greater flexibility than traditional stainless steel wires.

The available evidence about the efficacy of these newer wires is limited. Most of the included studies compared different wire types, but the quality of their comparisons and outcomes could have been better. One study did report that a conventional NiTi wire was no more painful than a superelastic NiTi wire, but only on day 1 of treatment evaluated using a visual analog scale (VAS) and by consumption of analgesics. Another study compared a conventional NiTi wire with a thermoelastic copper NiTi wire but reported no differences in the outcomes of pain and time to alignment, both measured by Little’s Irregularity Index, at three weeks, six weeks, and 16 weeks after the start of treatment.

During your first appointment, we will take X-rays and make molds of your teeth. We will also apply fluoride and protective gel to your gums.

We will use a special adhesive to bond the brackets to your teeth at your second appointment and install the archwire. We will then use ligatures (small elastic bands) to keep the archwire in place and guide each tooth into its new position. Ligature elastics come in a variety of colors and sizes. There are two types of ligature elastics: open coil springs and closed coil springs. Open coil springs help widen the gap between teeth, while closed ones can close a space between two teeth.

Straighter teeth are easier to brush and floss effectively, which can reduce the buildup of plaque and bacteria. This helps to lower the risk of tooth decay, periodontal disease, and bad breath.
